Where do COGS come from if my export does not carry them?

You set cost per product once, in a table you can paste from a spreadsheet, and every order recomputes against it. Shopify and WooCommerce exports that already carry cost per item are read directly.

Is this per order or per product?

Both, and the difference is the point. An order's margin averages every line on it; a product's margin follows that SKU across all 24,623 orders in the sample store, which is where the two SKUs under the 20% floor turned up.

Does it handle bundles and variants?

Variants are separate lines with their own cost and their own margin. Bundles are read as their component lines, so a bundle that loses money does not hide inside a healthy parent product.
